Lunar surface flown 8x10 inch sized index checklist "DPS" - pages 29+30 from the lunar surface flown "Apollo 15 LM Malfunction procedures Manual".
Printed on both sides !
This checklist stayed on the lunar surface inside the Lunar Module for a total of three days during the first extended scientific exploration of the moon at Hadley Apennine.
In addition to our own COA a notarized COA has been done here in Germany that comes with the checklist together with a full report (translated into English from German) by the public notary.
We have obtained the entire Malfunction checklist originally sold by astronaut David Scott in auction.

CUSTOMS / DUTIES / TAXES
International orders likely will have customs, duties and / or taxes imposed on your shipment, before receiving your package. All of these costs are the customers responsibility. We are required to fill out the forms when we ship your package. We ALWAYS lower the price, near or cost, as to help reduce what may be imposed and charged to our customers.
RETURNS:
Returns are accepted and given full refunds, if the item is returned in the original condition received. Refunds are then authorized and made to the method of payment that was made.
Special Orders, such as the Replica Apollo Suit cannot be cancelled, once ordered. Art Prints, Astronaut Suits and Models once shipped, cannot be cancelled. If returned, there is a 15% restocking fee. All orders older than 30 days that are returned for any reason, are also charged a 10% restocking fee.
